π Tests Needed Checklist & Screening Protocols
GOLD & USPSTF Standard
| Test | Target & Indication | Impact & Clinical Action |
|---|---|---|
| Spirometry with BD | Pre- & Post-BD FEVβ/FVC | Confirms fixed airflow obstruction in COPD (<0.70) or variable bronchodilator reversibility in asthma (>12% and >200 mL). Essential for staging. |
| Low-Dose CT (LDCT) | Annual Lung Cancer Screening (USPSTF 2021 / CMS) | Criteria: Age 50β80 years with ≥20 pack-year smoking history who currently smoke or quit ≤15 years ago. Reduces lung cancer mortality by 20β24% (NLST & NELSON trials). Shared decision-making visit mandatory. |
| Allergy Skin Prick (SPT) | Inhalant aeroallergen sensitization | Identifies immediate IgE-mediated reactivity to dust mites, pets, molds, pollens. Mandatory 3β7 day hold on antihistamines. In-office read in 15β20 minutes. |
| Serum Specific IgE (sIgE) | Quantitative blood allergen panel | Threshold ≥0.35 kU/L confirms sensitization. Unaffected by oral antihistamines or extensive eczema; safe in severe/uncontrolled asthma. |
| Total Serum IgE | Basal immune activation (IU/mL) | Required for Omalizumab (Xolair) dosing (range 30β1500 IU/mL). If >1000 IU/mL with central bronchiectasis, screen for Allergic Bronchopulmonary Aspergillosis (ABPA). |
| Exhaled Nitric Oxide (FeNO) | Airway eosinophilic inflammation (ppb) | >50 ppb (adults) indicates active Type 2 central airway inflammation and predicts high corticosteroid responsiveness; guides Dupilumab matching. |
| Blood Eosinophils (BEC) | Absolute blood count (cells/µL) | In COPD: ≥300 cells/µL identifies benefit from Inhaled Steroids (IMPACT/ETHOS) and Dupilumab (BOREAS). In asthma: ≥150β300 guides Anti-IL-5 (Benralizumab/Mepolizumab). |
| Alpha-1 Antitrypsin (AAT) | Serum AAT protein level & genotype | Universal one-time screening in all diagnosed COPD patients. Serum level <11 µmol/L (57 mg/dL) qualifies for weekly IV Alpha-1 augmentation therapy. |
| Arterial Blood Gas (ABG) / Oximetry | SpOβ ≤ 88%, PaCOβ > 53 mmHg | Resting PaOβ ≤ 55 mmHg (or SpOβ ≤ 88%) qualifies for Long-Term Oxygen Therapy (≥15 h/d). Daytime hypercapnia (PaCOβ > 53) post-flare qualifies for Home BiPAP. |

π Corticosteroid Stewardship & Tapering
GOLD & GINA EVIDENCE
β οΈ GUIDELINE RECOMMENDATIONS ON CHRONIC CORTICOSTEROIDS
COPD (GOLD 2025/2026 Mandate)
β’ Chronic Maintenance OCS is NOT Recommended: Long-term oral corticosteroids have no role in stable COPD (EVIDENCE A). Multiple randomized trials confirm no long-term reduction in exacerbations or mortality, while causing severe systemic toxicity (steroid-induced myopathy, osteoporosis, adrenal insufficiency).β’ Acute Exacerbation Exception Only: Systemic steroids are strictly indicated for acute exacerbations. The standard of care is Prednisone 40 mg PO daily for 5 days max (REDUCE Trial, EVIDENCE A). Longer courses offer zero incremental benefit and double adverse event rates.
β’ Chronic Inhaled Corticosteroids (ICS): Inhaled steroids should never be used as monotherapy. Restrict to patients with blood eosinophils ≥ 300 cells/µL (or ≥ 100 on LABA+LAMA with persistent exacerbations), always balanced against the dose-dependent risk of bacterial pneumonia (FLAME / IMPACT NNT 3β4 / ETHOS NNT 3.2).
Asthma (GINA 2025/2026 Mandate)
β’ Maintenance OCS is Strictly a Last-Resort Add-On: In Step 5, maintenance low-dose OCS (≤ 7.5 mg/day prednisone-equivalent) is restricted to patients failing high-dose MART + LAMA and who are ineligible for or fail targeted biologics (EVIDENCE D).β’ OCS Stewardship & Steroid Elimination: The primary modern standard of care in severe asthma is to replace chronic OCS with phenotype-guided biologics (Anti-IL5, Anti-IL4Rα, Anti-TSLP, Anti-IgE) to achieve complete steroid cessation.
β’ Threshold for Irreversible Harm: Cumulative systemic steroid exposure exceeding 0.5 to 1.0 gram of prednisone equivalent per lifetime/year triggers accelerated cataracts, fractures, diabetes, and cardiovascular events.

π©Έ TOXICITY MONITORING & TAPERING PROTOCOL
Systemic Corticosteroid Organ Toxicity Checklist
| Organ System | Complication | Proactive Monitoring & Mitigation Protocol |
|---|---|---|
| Adrenal (HPA) | Secondary Adrenal Insufficiency | Check morning 8:00 AM serum cortisol before dose/taper. Consider cosyntropin stimulation if morning cortisol 3β15 µg/dL. Provide emergency hydrocortisone kit and medical alert ID. |
| Skeletal | Osteoporosis & Vertebral Fractures | Baseline DEXA scan if cumulative exposure > 3 months. Prescribe Calcium (1000β1200 mg/day) + Vitamin D (800β2000 IU/day); initiate oral bisphosphonates or Denosumab if high FRAX risk. |
| Metabolic | Steroid-Induced Diabetes & Weight Gain | Baseline HbA1c; monitor fasting glucose and 2-hour postprandial levels during courses; blood pressure checks at every visit. |
| Ophthalmic | Posterior Subcapsular Cataracts, Glaucoma | Annual slit-lamp exam and intraocular pressure (tonometry) measurement for anyone receiving maintenance OCS or high-dose ICS for > 6 months. |
| Muscular | Proximal Steroid Myopathy | Screen for bilateral pelvic girdle/quadriceps weakness and difficulty climbing stairs. Distinguish from COPD deconditioning (myopathy spares distal reflexes and sensation). |
| Gastrointestinal | Gastritis & Peptic Ulcer Disease | Proton pump inhibitor (PPI) or H2 blocker co-prescription if combined with NSAIDs, aspirin, or anticoagulants. |
Stepwise Tapering Guidelines (Post-Biologic Initiation):
β’ Phase 1 (Dose > 10 mg/day Prednisone): Reduce daily dose by 2.5 to 5 mg every 1 to 2 weeks while patient remains stable and free of acute asthma symptoms.
β’ Phase 2 (Dose 10 mg to 5 mg/day): Reduce daily dose by 1 to 2.5 mg every 2 weeks.
β’ Phase 3 (Dose ≤ 5 mg/day physiological replacement): Switch to Hydrocortisone 10β15 mg in the morning + 5 mg in early afternoon OR taper Prednisone by 1 mg every 2 to 4 weeks.
β’ Check 8:00 AM serum cortisol before morning dose:
β’ Cortisol < 3 µg/dL: Severe adrenal suppression; continue physiological replacement, recheck in 3 months.
β’ Cortisol 3β15 µg/dL: Partial recovery; perform short Synacthen / ACTH stimulation test.
β’ Cortisol > 15 µg/dL (or post-ACTH peak > 18 µg/dL): Intact HPA axis; discontinue oral steroid safely.
